Late presentation of colorectal cancer in a vulnerable population

This month's issue of the American Journal of Gastroenterology investigates late presentation of colorectal cancer in a vulnerable population.

Dr Chanda Ho from California, USA examined colorectal cancer stage at presentation and mortality in a vulnerable population compared with nationally representative data.
 
Colorectal cancer cases were identified from San Francisco General Hospital and the Surveillance Epidemiology and End Results database.
 
The research team identified that 55% of the San Francisco General Hospital cohort presented with advanced disease, compared with 44% of the Surveillance Epidemiology and End Results cohort. 

Increased risk of advanced stage at presentation at San Francisco General Hospital compared with Surveillance Epidemiology and End Results was most evident among blacks and Asians.

The doctors noted that there was weak evidence for worse survival at San Francisco General Hospital compared with Surveillance Epidemiology and End Results overall. 

This varied by race with poorer survival at San Francisco General Hospital among whites and possibly blacks but some evidence for better survival among Asians. 

The doctors found that among colorectal cancer patients at San Francisco General Hospital, Asians and Hispanics had better survival than whites and blacks. 

At San Francisco General Hospital, 44% had a diagnosis of colorectal cancer within 1 year of establishing care there. 

Of those who had established care at San Francisco General Hospital for at least 1 year, only 22% had exposure to colorectal cancer screening tests.

Dr San's team commented "These findings allow examination of colorectal cancer presentation by ethnicity in vulnerable populations and identify areas where access and utilization of colorectal cancer screening can be improved."
