Posttransplant diabetes mellitus after pancreas transplantation has not been extensively examined.
Dr Neidlinger and colleagues from Wisconsin, USA conducted a single center, retrospective analysis of 674 recipients from 1994 to 2005.
The research team examined the incidence of and risk factors for posttransplant diabetes mellitus after pancreas transplantation.
Posttransplant diabetes mellitus was defined by fasting plasma glucose level of 126 mg/dL.

The researchers found that the incidence of posttransplant diabetes mellitus was 14%, 17% and 25% at 3, 5 and 10 years after pancreas transplantation, respectively.
The incidence of posttransplant diabetes mellitus was higher in solitary pancreas versus simultaneous kidney pancreas recipients.
In multivariate analysis, factors associated with posttransplant diabetes mellitus included older donor age, higher recipient body mass index, donor positive/recipient negative CMV status, and posttransplant weight gain.
The team noted that additional factors associated with posttransplant diabetes mellitus included pancreas rejection and 6 month fasting glucose, hemoglobin A1c and triglyceride to high-density lipoprotein ratio.
Dr Neidlinger's team concluded, “This study delineates the incidence and identifies risk factors for posttransplant diabetes mellitus after pancreas transplantation.”
