A 40 year old man admitted with small bowel obstruction and a past history of
Crohns. Not on any current treatment for Crohns. Present problem thought to
be due to a flare up of the Crohns and started on steriods. Obstruction worsened and a plain X-ray showed massively dilated loops of small bowel with free gas in the wall of the bowel and the mesentery. Went to emergency
surgery. Obstruction found to be caused by strictured areas of burnt-out
Crohns with no active disease. Stricturoplasties performed. Gas noted in wall of bowel and mesentery. No evidence of ischaemia.
