Paraoesophageal (rolling) hiatal hernia.
This is less common than sliding hiatal hernia. With the endoscope in the J-position in the stomach the apex or fundus of the hernia is seen in this figure to the right of the endoscope. Paraoesophageal hernia may become incarcerated or even strangulated. Also, it predisposes to gastric volvulus.
