Colonic diaphragm disease.
This 72-year-old woman on long-term slow-release diclofenac (SR-D) for non-specific arthritis presented with 10 kg weight loss, diarrhea and normocytic anemia (Hb-9g/dl) with an ESR of 103, leucocyte count of 18.3, CRP of 91mg/l, platelet-count of 600 and positive FOBs. Oesophagogastroduodenoscopy (OGD) was non-diagnostic. Colonoscopy revealed benign discrete ulcers and a diaphragmatic stricture in the colon. Withdrawal of NSAID led to dramatic clinical improvement, with 12 kg weight gain, disappearance of arthritic pain, resolution of anemia, and normalization of inflammatory markers.
