A 63-year-old man with long-standing achalasia. At the time of the current presentation, it would take him an hour to eat a meal. A contrast study 20 years earlier had shown typical changes of achalasia, but he had always declined treatment.

On this occasion he presented with tiredness and lethargy and was found to have a carcinoma of the caecum. The CT scan of the chest (performed as part of the work-up for his colon tumor) shows a grossly dilated esophagus, full of food.
