Ulcerative colitis (advanced).
(a) The plain film shows a grossly contracted, ribbon-like, colonic gas shadow, which is shortened, completely lacking in a haustral pattern and irregular in outline. (b) A SCBE on the same patient also shows a grossly contracted colon with no haustration, but with multiple filling defects. These pseudopolyps are due to islands of inflamed mucosa in an otherwise denuded bowel wall.
