Duodenal scarring secondary to peptic ulceration.
In this patient, with a long history of periodic dyspepsia, the normal anatomy of the pyloroduodenal region is no longer recognizable because of gross deformity resulting from scar tissue, and there is a persistent stenosis (arrowheads) unrelieved by Buscopan.
