An 83-year old woman presented with an acute confusional state, pyrexia and jaundice. Abdominal ultrasonography showed dilatation of the intrahepatic biliary tree. ERCP revealed a tight stricture in the proximal common bile duct through which a guide wire has been passed. The stricture was brushed for cytology and a stent was inserted with drainage of pus. The brushings revealed abnormal cells highly suggestive of a cholangiocarcinoma.
