Classical Cases

Click on the link above to send your submission by e-mail. Attach your text as a Word document and any illustrations as jpg files.

In your Word document, list the content of the report under the following headings:

• Title, Author, E-mail address, Department, Institution, City, Country
• Summary (maximum 100 words)
• Case Report, including the subheadings History, Examination, Investigation, Management, and Outcome (maximum of 750 words)
• Discussion (maximum 500 words)
• References (maximum of 10 references)

Figures
Figures should be in JPEG format (.jpg), with a maximum file size of 500 KB and a minimum resolution of 300 dpi (and if possible, color images saved as CMYK).

Portrait
Please submit a head-and-shoulders picture of yourself (as a JPEG) to accompany the Classical Case.

Copyright
By submitting your Classical Case for publication, the copyright will be assigned to Blackwell Publishing Ltd. Please download and return the copyright assignment form below as confirmation of this assignment. Complete the copyright assignment form.
